This is figure 1-34 of the Apollo 12 Preliminary Science Report, which has the following caption:
Astronaut Bean and two U.S. spacecraft on the surface of the Moon. This photograph and figure l-29 clearly show how close Astronaut Conrad landed the LM to its predesignated landing point. After completing their work around Surveyor 3, the crew moved back toward the LM.
